Clinical Psychology of Fort Smith, LLC offers a wide variety of services to help you achieve better mental health – for life. Our clinical and administrative staff are committed to making your experience with us beneficial and meaningful.

On your first visit, you will meet with one of our expert mental health professionals for an in-depth discussion of your concerns. At the end of your first visit, our practitioners will develop an individualized treatment plan designed to meet your unique needs. This treatment plan will include specific recommended treatments and suggested frequency of sessions. Additionally, we will also coordinate your mental health treatment with your other healthcare providers, with your permission. We look forward to working with you toward the goal of living a healthier, happier life.

Before your first visit, you will be asked to provide personal information and insurance details necessary to provide you the best care while limiting your out-of-pocket expenses as much as possible. You can download the necessary forms by clicking the appropriate files below. Note: There is a forms for adult patients and a form for child patients, in both PDF and Word Doc format.

You can either email them back to us at info@fortsmithpsychology.com or bring them with you to your first appointment. While it is not required, we suggest providing insurance information prior to your arrival for the first appointment to avoid delays if there is any difficulty in verifying your benefits.

Scheduling Your Initial Consultation

Just call our main number, (479)783-0445, and one of our administrative staff will be happy to schedule an appointment for you to meet with one of our mental health professionals.

Privacy Policy

The law protects the privacy of all communications between a patient and a psychologist and, in most situations, information regarding your treatment can only be released to others with your signed consent. A copy of our Clinic Policies and Practices will be provided to all patients clearly outlining how psychological and medical information may be used. We encourage all patients to review it carefully.

Patient Services Agreement

In addition to the Notice of Policies and Practices, we will also provide you with information regarding our services. This includes important information about the Health Insurance Portability and Accountability Act (HIPAA), cancellation policies, our professional fees and contact information.

Rates and Insurance

Clinical Psychology of Fort Smith, LLC will be happy to provide a current fee schedule upon request. In addition to patient appointments, it is our policy to charge for other necessary professional services including report writing, phone conversations lasting longer than 15 minutes and, with your permission, consulting with other professionals.

Insurance benefits can be applied towards our services by providing the necessary information. Patients are encouraged to contact their insurance company regarding outpatient mental health benefits and requirements. All initial authorizations, certifications and/or referrals are the responsibility of the patient and will need to be in place prior to their first appointment. Our office staff will verify any benefits at the time of the initial consultation and any co-insurance, co-payment and/or deductible will be due at the time of service.

It is office policy that all costs incurred will ultimately be the responsibility of the patient or parent/guardian if the patient is a dependent. Therefore, it is strongly advised that all patients explore all aspects of their insurance coverage and limitations. Please be sure to bring any necessary paperwork with you to your appointment.

If your appointment should in any way involve pending or future litigation, please call the office prior to the appointment as the psychologist will want to speak with you directly.
