Welcome to Clinical Psychology of Fort Smith

We look forward to working with you toward the goal of “making your life better.” Our clinical and administrative staff are committed to making your experience with CPFS beneficial and meaningful. On your first visit, you will meet with one of our psychologists for an in-depth discussion of your particular concerns. At the end of this initial consultation, your psychologist will develop with you an individualized treatment plan designed to meet your concerns. This treatment plan will include specific recommended treatments and frequency of sessions. With your permission, we will also coordinate your mental health treatment with your other health care providers.

During this first visit, you will also be asked to provide personal and insurance information necessary to provide you the best care while limiting your “out of pocket” expenses as much as possible. You can download the necessary registration forms by clicking in the box above. You can then either fax them to us or bring them with you to your initial consultation. You may also provide your insurance information prior to your arrival for the first appointment. Doing so will help to avoid delays if there is any difficulty in verifying your benefits.

Privacy Policy

The law protects the privacy of all communications between a patient and a psychologist and, in most situations, information regarding your treatment can only be released to others with your signed consent. A copy of our Clinic Policies and Practices will be provided to all patients clearly outlining how psychological and medical information may be used. We encourage all patients to review it carefully.

Patient Services Agreement

In addition to the Notice of Policies and Practices, we will also provide you with information regarding our services. This includes important information about the Health Insurance Portability and Accountability Act (HIPAA), cancellation policies, our professional fees and contact information.

Rates and Insurance

CPFS will be happy to provide a current fee schedule upon request. In addition to patient appointments, it is our policy to charge for other necessary professional services including report writing, phone conversations lasting longer than 15 minutes and, with your permission, consulting with other professionals.

Insurance benefits can be applied towards CPFS services by providing the necessary filing information. Patients are encouraged to contact their insurance company regarding outpatient mental health benefits and requirements. All initial authorizations, certifications and/or referrals are the responsibility of the patient and will need to be in place prior to their first appointment. Our office staff will verify any benefits at the time of the initial consultation and any co-insurance, co-payment and/or deductible will be due at the time of service.

It is office policy that all costs incurred will ultimately be the responsibility of the patient or parent/guardian if the patient is a dependent. Therefore, it is strongly advised that all patients explore all aspects of their insurance coverage and limitations. Please be sure to bring any necessary paperwork with you to your appointment.

If your appointment should in any way involve pending or future litigation, please call the office prior to the appointment as the psychologist will want to speak with you directly.
